AT THE 2017 NORTHEAST REGIONAL CONFERENCE
It recognizes that while an idea to achieve better patient and family outcomes may start with one individual, it often takes an entire team/chapter to implement successfully. The DAISY Team Award is designed to honor collaboration by two or more people, led by a nurse, who identify and meet patient and/or community needs by going above and beyond the traditional role of Nursing.

This is a certificate of appreciation that recognizes a chapter of the organization that is committed to the building of programs, projects, and partnerships. The RAM Project was recognized as the Eta Eta Chapter's project.
Eta Eta Chapter
The Eta Eta Chapter of Chi Eta Phi Sorority, Incorporated. was organized April 14, 1984 in Petersburg, VA at Petersburg High School. Annually, we have provided scholarship funds for nursing students locally and/or nationally; made donations to other non-profit organizations such as United Negro College Funds, nationally, CARES shelter for women and children, and Mary Carter Beacon House, locally.
We have supported or provided community programs and projects to citizens in the Tri-City area and surrounding counties, for individuals, Health Fairs, Relay for Life cancer project, VSU Cares project with the Dental Association and Mission of Mercy Charity Organization. We have provided our own educational programs with Black History, Heart Diseases, Hypertension, Stroke, Diabetes, and other Disease Prevention programs in the areas. And, we maintain membership with the
National Council of Negro Women.
